Kindergarten
All kindergartners are required by the State of Illinois to submit:

If your child brings donated clothes home, please launder and return them to the Health Office.
Lice: It's a great idea to wear your hair up (pony, pigtails, braids). Keeping your hair off of others helps prevent the spread of head lice. If Lice or nits are found, the student will be sent home for a treatment. Please come to the health office for a re-check when nit-free.

"Vision screening is not a substitute for a complete eye and vision examination by an eye doctor. Your child is not required to undergo this vision screening if an optometrist or ophthalmologist has completed and signed a report indicating that an examination has been administered within the previous 12 months." NOTE: This is NOT a request for permission to screen. Permission to screen is not required as vision and hearing screening is a state mandate. |

If your student will be taking medication at school; inhaler, epi pen, over the counter and or prescription medications you and your physician will need to complete this form.
